Purpose: Define usage and outcome measurement for each tool before deployment. Usage metrics tell you whether the tool is being used. Outcome metrics tell you whether it is improving performance. Both are required. Design measurement before deployment — retrofitting is unreliable.
Measurement Layers Reference
Layer 1 — Usage metrics (leading indicators): Access rate, timing, completion rate. Tell you whether the tool is being used. Do NOT tell you whether it is working.
Layer 2 — Outcome metrics (effectiveness): Performance quality at the supported moment, error rate, escalation rate, L3 behavior observation. Tell you whether the tool is changing performance.
