Purpose: Rate each assumption from Tool 1 on Risk if Wrong × Testability before launch. Assign the priority action. High-risk assumptions without a testing plan or monitoring watchpoint are program launch blockers.
Priority Action Reference
High Risk × High TestabilityTest before launch — required. Adjust design based on findings. This is a launch blocker if untested.
High Risk × Medium TestabilityTest what is testable. Document residual uncertainty as a monitoring watchpoint with a named signpost.
High Risk × Low TestabilityBuild explicit monitoring infrastructure. Pilot small-scale before full deployment. Named owner required.
Medium Risk × Any TestabilityDocument and monitor. Test in pilot if feasible. Monitoring watchpoint recommended.
Low Risk × Any TestabilityDocument as a noted assumption. Proceed. No testing required.
LAUNCH BLOCKERAny High Risk assumption without a testing plan OR a monitoring watchpoint with a named owner. Must be resolved before GO decision.
